· Serves as primary liaison for guardians/families and external stakeholders.
Works to resolve problems or situations that arise during placement.
· Educates families and external stakeholders about the treatment approach, philosophy, and expectations for parent/guardian involvement.
· Documents and communicates sensitive and confidential information according to policy in a professional and timely manner.
· Ensures documentation of services such as: consents, releases, intake procedures, referral contacts, placement plans, treatment, discharge reports, and records are completed in a timely manner according to federal, state, and agency policy and procedures.
· Ensures financial and clinical eligibility for services prior to delivery of such services.
Discusses and assists in making financial arrangements with parents/guardians and other agencies, makes referrals to financial resources, and procures funding commitments.
· Completes notes and billing for CM services within required timeframe.
· Leads and coordinates all activities for referral (depending on the service area), admission, assessment, intake, patient rights, child abuse and neglect reporting, and discharge in conformance with WCS policies and procedures.
· Assumes responsibility for coordinating the overall design, implementation, and documentation of treatment, ensuring that the objectives are aligned with the treatment plan, treatment direction, and stage of treatment for the child and assuring that progress is being made and documented.
· Leads the treatment team and provides assistance to external entities in identifying and resolving external issues impacting treatment.
· Coordinates with guardians, community agencies, and professionals to ensure a cohesive approach throughout treatment is reported and discussed.
· Takes lead in the coordination and facilitation of all meetings, in-take, staffing, and case conferences and ensures the treatment team thoroughly assesses plans, evaluates, reviews, and revises each child’s treatment direction and plan.
· May serve as an agency representative during custody hearings, placement, or other judicial procedures.
· Works with team to identify and develop new referral sources as well as maintaining and strengthening relationships with existing referral sources.
· Attends in-service training, conferences, seminars, etc.
, to maintain knowledge of current trends and developments in the service delivery of treatment to children and their families as well as of information, standards, and protocols regarding Montana Medicaid/Mental Health Services Plan.
· May make recommendations to parents/guardians for other agencies or facilities for treatment services when child/youth qualifies for and would benefit from further treatment.
· Monitors the day-to-day delivery of services and takes the lead in collaborating with other providers to ensure the coordination of all treatment team efforts.
· Completes and maintains in an accurate and timely manner a variety of records and reports in accordance with federal, state, and local laws, program policy, and Medicaid regulations.
· Seeks and refers parents to parent education and training toward skill development with techniques for child management, problem-solving, practical everyday living, and increasing the sense of safety and nurturance within the parent/child relationship.
· Provides consultation, training, and support for public school personnel in efforts to strengthen their capacity to work successfully with the child.
· Discusses and coordinates transportation needs, medication management, and therapeutic interventions needs when appropriate.
