Case Manager

Job Introduction: Maximus currently has a Case Manager opportunity with the Maximus Montana Employment and Training Project.
This Project will provide intake, assessment and development of an Employability/Service Plan (ESP) plus case management of participants along with job development, placement, & retention services.
Our goal is to equip participants to be successful in the world of work by building their skills capacity, prioritizing employment, and supporting retention.
Job Summary: Essential Duties and Responsibilities - Promote working relationships with customers and monitor engagement and progress.
- Liason with customers on a regular basis to meet and achieve program goals and/or maintain program services and eligibility.
- Exhibit considerable programmatic knowledge and assist customers in multiple phases of the application process, ranging from enrollment through the awarding of benefits and services.
- Assist customers to acquire services that facilitate program goals (e.
g.
, educational and/or vocational training, medical, child care, transportation, substance abuse/mental health, child support establishment, legal, and other related needs).
- Maintain accurate and timely case notes on all customer contacts and document activities.
- Share information about outreach and engagement efforts with project staff.
- Inform project staff of any barriers that they identify and that are preventing the customer from engaging with the Provider.
Minimum Requirements - High School Diploma - 2-4 years of experience Education and Experience Requirements: This role is contingent upon an award of the contract.
This position requires travel up to 10% of the time.
This position requires you to have reliable transportation to travel to other offices within the region.
