Luke Community Healthcare is expanding our population health program and seeking a civic-minded Community Health Worker (CHW) who is passionate about the health and well-being of our community.
The CHW collaborates with medical providers, primary care teams and social services agencies.
Playing a key part of a care coordination team to provide short-term care and connection to resources and support to program clients.
Helping to improve their health and general well-being through direct communication and provision of care and services.
Works in both clinical and community- based settings.
The ideal candidate would possess the knowledge and willingness to learn about cultural components of health and wellness in our area.
Any combination of 3 years health/social services experience and/or education is preferred.
The candidate would obtain a Community Health Worker Certification within 1 year of employment.
Must have the ability to work with patients and provide strong data analysis skills.
