Partners In Home Care is the premier home health agency in Montana.
With a staff complement of 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, social work and bereavement support.
Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid “waiver” program that supports those who wish to live with independence).

We are seeking two full-time Social Workers who have their MSW to work in our Hospice Department.

help Hospice and Home Health patients and their families to deal more effectively with the psychosocial implications of their illnesses through counseling, education and referrals to community resources.
You'll work with an inter-disciplinary team that puts the patient at the center of care focused on assisting recovering patients or end-of-life.
Use your counseling skill to help patients and families with information, education and community resources.
You may also be called upon to provide grief and bereavement counseling in coordination with our Bereavement Coordinator and Chaplains.
Masters degree in Social Work from an accredited institution, a valid driver’s license, clean driving record, appropriate auto insurance, and reliable transportation.
You'll need high speed Internet needed for remote work.
We prefer that you have one year of previous experience as a social worker in a medical setting and working knowledge of Medicare/Medicaid regulations, death and grief issues, Conditions of Participation and community resources.
We are looking for two very special folks who will work well with patients at end of life and their families.
