We are currently seeking the right individual to join our team as an MDS Coordinator to our Skilled Nursing neighborhood.
We are looking for an MDS Coordinator who is patient, tactful, organized and genuinely interested in enriching the lives of seniors, and will: • Be compassionate and caring; • Value being part of the skilled nursing team; • Be able to observe resident behavior and communicate the observations in a professional and intelligent manner; • Accurately and timely complete MDS assessments; • Assist with admissions and discharges of residents and patients; • Lead resident care conference meetings with residents and families; • Evaluate and document the effectiveness of all nursing services; • Evaluate the effectiveness of patient care plans; • Provide support to the licensed nurses and certified nursing assistants of the skilled nursing team.
Qualifications: • Nursing Degree from an accredited school of nursing.
• Possess a current RN or LPN license in the state of Washington.
• MDS experience needed • CPR/BLS Certified • Ability to work occasional weekend and/or holiday shift • Working with older adults in a health care setting; geriatric mental health is a plus.
