Partners In Home Care, a highly-respected home health agency based in Missoula, Montana is seeking to hire a few folks who want to reinvigorate their nursing career working with a highly professional healthcare team.

With a staff complement approaching 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, social work and bereavement support.
Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid “waiver” program that supports those who wish to live with independence).
We are seeking LPNs to work with our Home Health patients.
Under the supervision of an RN Case Manager, LPNs travel to patient homes to provide high quality nursing care along side the Clinical Care Team members, which could include nursing, physical therapy, occupational therapy, social work and more.
We assign you to a multi-disciplinary team based upon geographic location.

LPNs provide skilled nursing care for assigned patients in their place of residence; coordinate and integrate services to ensure continuity of care; facilitate the achievement of quality, clinical and cost outcomes; create opportunities and systems to enhance outcomes; and use the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation through an inter-disciplinary team approach.
Essential Duties and Responsibilities: Provides skilled nursing and coordinates care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician.
Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions and referrals.
Observes, assesses, evaluates, interventions in relation to patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications.
Supports continuity of care through effective communication, information exchange with Case Manager.
Maintains accurate, current and complete patient records in accordance with Agency standards and requirements as well as knowledge of current reimbursement rules and regulations.
Participates in quality improvement activities and attends in-service training as assigned.
Your Qualifications.

Required: A current LPN license in the State of Montana (BSN preferred), a valid driver’s license, appropriate auto insurance, and reliable transportation.
Preferred: 1-2 years of recent acute care or home care experience.

LPN staff do not participate in the on-call rotation.
