Job Summary:
The Social Worker empowers Hospice and Home Health clients and their families to cope more effectively with the psychosocial implications of their illnesses through counseling, education, validation, and referrals to community resources.
Serving as advocates; addressing individual and family systems to establish therapeutic avenues for support.
Collaborates effectively with all team members regarding both the client and family, ensuring support, care, and satisfaction.
Essential Duties and Responsibilities:
1.
Cross-trains in Home Health to ensure seamless knowledge of patients transitioning from Home Health to Hospice, offering consistency of care, education and support with end-of-life.
2.
Assesses and evaluates psychosocial and emotional factors of Hospice and Home Health clients and their families, addressing all aspects of functioning, completing a comprehensive needs assessment with follow-up support.
3.
Counsels clients and families on the adjustment to terminal illnesses, addressing family systems, mental health factors, environmental stressors and end-of-life needs.
4.
Instructs on availability of, and provides referrals to, appropriate community resources.
5.
May provide grief and bereavement counseling to clients and their families.
6.
Provides pertinent information related to the psychosocial status of clients and their families during team meetings and whenever indicated to facilitate team communication and collaboration.
This would also include collaboration with outside agencies as needed.
7.
Facilitates groups pertaining to end-of-life and provides education regarding Hospice and other topics to outside agencies and communities.
8.
Attends interdisciplinary team, social work, all-staff, and relevant meetings as indicated.
9.
Performs other duties as assigned by supervisor.
Minimum Qualifications:
Qualifications include BSW or MSW from an accredited institution, a valid driver’s license, appropriate auto insurance, reliable transportation and high-speed Internet access from home.
One year of previous experience as a social worker in a medical setting preferred.
Working knowledge of Medicare/Medicaid regulations, death and grief issues, Condition of Participation, and community resources preferred.
