Job Description: Mental Health Case Manager in Missoula, MT, US Are you passionate about making a difference in the mental health field and serving your local community? Join our team as a Case Manager and become a valuable advocate for those we support.
Your role will involve being a knowledgeable spokesperson and guiding individuals through their unique mental health journey.
Assist clients in obtaining and maintaining government benefits will be one of your key responsibilities.
You may also attend court hearings and provide crucial information about clients' specific diagnoses.
A bachelor's degree in Social Work, Psychology, or a related field is required for this position.
Preference will be given to applicants with 1-2 years of experience working with clients with severe mental illness (SDMI).
Compensation: $19 hourly Responsibilities: Assess the needs of clients, including their activities of daily living, every 90 days.
Facilitate referrals and provide access to resources and service providers to meet clients' needs.
Collaborate with clients and anyone they feel should be involved in planning treatment, at least every 90 days.
Represent Winds of Change in a professional manner.
Document progress notes on a daily basis.
Complete internal and external forms and other documents promptly.
Participate in required training, whether online or in person.
Possess a valid driver's license and have transportation and automobile insurance.
Adhere strictly to HIPAA rules and maintain confidentiality of client information.
Maintain productivity standards.
Attend meetings or events as needed on a weekly basis.
Ensure consistent attendance.
Qualifications: Familiarity with and adherence to HIPAA regulations.
Ability to maintain a high level of confidentiality and discretion.
Proactive approach and ability to anticipate client needs.
Demonstrates sound judgment and accuracy in decision-making.
Displays passion for the work and maintains a positive attitude.
Takes personal responsibility for actions, keeps commitments, and seeks/offers help when needed.
Exhibits professional appearance and hygiene in line with workplace culture.
