Job Summary:
The Wait List Social Worker maintains program wait lists, coordinates and performs assessments, and arranges services for members in the Home & Community Based Services (“HCBS”) program.
Essential Duties and Responsibilities:
Maintains current documentation of client-related activities, program statistics, and scheduling.
Acts as liaison for HCBS contract providers, community agencies, and prospective members.
Maintains wait lists for two Medicaid Waiver programs, including performing wait list assessments, and reviewing referrals
Prepares paperwork related to client care according to Department of Public Health and Human Services regulations, which includes verification and documentation of Medicaid eligibility.
Coordinates acuity visits, including scheduling, conducting in-home visit, determination of potential safety issues, and follow-up documentation.
Establishes and maintains accounts, ticklers, logging, scheduling, client charts, filing, and referral systems.
Prepares periodic statistical reports for State and agency purposes as directed.
Facilitates member enrollments and resolves issues for Medicaid.
Performs general office tasks, including processing documents, filing, ordering supplies, attending meetings, taking minutes, conducting research and covering telephones.
Performs other duties as assigned by supervisor.
Minimum Qualifications:
Qualifications include bachelor’s degree in social work or related behavioral science, a valid driver’s license, appropriate auto insurance and reliable transportation.
One year of previous experience as social worker in a health care setting required.
Strong communication skills required.
Working knowledge of community resources preferred.
Physical Demands & Working Conditions:
Work is performed in both the office environment and in the residences of patients, requiring driving in varied weather conditions.
Work involves frequent sitting and standing, with occasional walking.
The position also includes regular use of a keyboard, and use of hands to finger, handle, and feel.
Position involves occasional exposure to blood and bodily fluids.
