Job Summary:
The Home Health Aide works under a client’s plan of care, with direct nursing supervision, to assist with or perform specific tasks associated with activities of daily living.
Essential Duties and Responsibilities:
Travels to clients’ residences in personal vehicle in varied weather conditions to perform assigned duties.
Schedule may include visits on weekends and coverage for absences of other caregivers.
Assists clients in a variety of personal care tasks and provides support for activities of daily living, based on individual needs as outlined in the plan of care, including the following:
Personal care needs such as bathing, grooming, dressing, and care of the mouth, skin, and hair
Bowel and/or bladder requirements, bed pan routines, and incontinence care
Medication monitoring (not including administration)
Ambulation assistance or assistance with exercises
Nutrition and meal planning, meal preparation, and feeding
Grocery shopping and general household tasks
Charts and records client information and care provision accurately and in a timely manner.
Reports all changes or irregularities in client or client’s environment to supervisor or case manager.
Practices basic infection control and maintains a clean, safe and healthy environment.
Performs other duties as assigned by supervisor.
Minimum Qualifications:
High school graduate or equivalent, a valid driver’s license, appropriate auto insurance and reliable transportation.
Must be licensed as a Certified Nursing Assistant
Physical Demands & Working Conditions:
Work is primarily performed in the residences of patients, requiring driving in varied weather conditions.
Occasional medium lifting is necessary.
Position requires prolonged standing and working in a variety of positions, including bending, stooping and kneeling.
The position includes use of hands to finger, handle, and feel.
Position involves frequent exposure to blood and bodily fluids.
